Please complete the forms found in the file below and bring them all with you to Oak Tree Surgery Center on the day of your scheduled procedure, along with your insurance card and your Photo ID.

 

You MUST have a RESPONSIBLE ADULT (at least 18 years old) to drive you home following your procedure. Patients will not be permitted to take a taxi home after having anesthesia.

 

For Women 55 years old and under, please be advised that you will be asked to provide a urine specimen prior to your procedure.

 

 

 


 

Insurance & Billing

Oak Tree Surgery Center accepts most major insurances, however it is important that you contact our billing department (732-902-2222) and your insurance company prior to receiving services or care at our facility. Each plan is different and some provide different levels of coverage that could make a difference in your out-of-pocket costs.

Our surgery center is dedicated to providing consumers with as much information as possible regarding potential healthcare costs. Accordingly, we have listed information on the insurers and providers the center participates in.

This list may not be comprehensive. For example, specialty insurers such as workers' compensation and personal injury protection ("PIP") are not included because there are a number of different types of insurance products and we may not participate in all of them. If you do not see your plan listed on our website, we strongly recommend that you contact us or your health insurance plan to confirm whether your plan is in-network or out-of-network.

Additionally, the physicians who provide care within our center might not participate in the same insurance plans as the surgery center. You should check with the physician who is arranging your procedure to see which insurance plans the physician participates in. You should also know that these healthcare professional(s)' costs are not included in the facility's charges; they will bill separately. We recommend that you contact your health insurance plan for further consultation on these costs.

About Your Bill

Surgery Center BillingOur surgery center billing department staff will call your insurance company prior to your procedure and verify the information you've provided and your medical benefits for our facility charges. We will need to secure any information regarding co-payments, co-insurance and/or deductible amounts that will be your responsibility. Payments that you are responsible for are expected in full prior to your procedure date or at the time of when you register. Please bear in mind, information provided prior to your visit is only an estimate based on the information provided by your surgeon and insurance company and we cannot tell you what your policy will cover. Upon request, a list of the center's standard charges for services can be provided.

 

Our facility practices Global Billing. This means that the bill sent to your insurance covers your preoperative evaluation, a majority of supplies and medications, equipment, personnel and use of the operating and recovery rooms. Should the final patient responsibility assigned by your insurance carrier be more than the pre-procedure estimate, you will receive a statement from the facility for the difference. Should your pre-surgery estimate have been higher than the final patient responsibility assigned by your insurance carrier, we will issue a refund to you within 30 days of receipt of payment from your insurance company. A full account ledger can be provided after your procedure at no additional charge.

 

If you do not have insurance or if your insurance does not cover the procedure to be performed, please make arrangements to pay the surgery center's facility fee prior to your date of service, or at the time of registration.

 

For your convenience, we accept cash, personal checks, VISA, MasterCard, Discover and American Express.

 

Anesthesiologist Billing - You will receive a separate anesthesia bill. The anesthesia services are not included in the surgery center fee. The anesthesia billing department will accommodate the patient by first billing the insurance company. However, any balance that is not covered by the insurance company will have to be met by the patient. There may be certain procedures where the anesthesia fee will be collected in advance. Co-payments, deductibles, etc. may also be collected in advance of the surgery.

 

Surgeon Billing - The surgeon will generate a charge for their services. Please contact your surgeon’s office to discuss any billing matters.

 

Other Billing - There may be other bills that a patient can incur during surgery. These are ancillary fees that may be utilized during your procedure (ex: pathology fees).

Other Care Providers

We work with other providers as part of your care. In addition to a bill from the facility and the physician, you may receive a bill from another provider, depending on the nature of your procedure.
